Before admission into the program, parents are required to fill out the registration forms and provide any additional information pertaining to the health and safety of their child. The following information is required to complete the registration:

  • Child's name
  • Child's allergy information
  • Child's doctor's information
  • Child's updated immunization record
  • Parent/Guardian information
  • Emergency contact
  • Custody information
  • Consent forms (available in the parent handbook.)
  • Policy agreement
  • Financial agreement
  • Weekly schedule form (if you require part-time care.)

All information obtained is strictly for the use of Where Children Grow, Young Achievers Daycare and on strict occasions for other professionals (i.e. Eastern Ontario Health Unit, Ministry of Education.) These professionals supervise our programs to ensure we are meeting the highest standards of childcare. The school or any other persons cannot receive any information that you have given us without your consent.


A registration fee applies to any family registering with Where Children Grow or Young Achievers Daycare. If you are registering for both programs, you will only be required to pay one registration fee. The registration fee is per family and is renewed on an annual basis.

The cost for register is $20.00 per family.
